Building a Safer Future Through Incident Analysis

Safety incidents present some of our best opportunities to improveWhile we establish processes and systems to identify risk before an incident occurs, we would be remised if we didn’t do a great job of learning when an incident occursThroughout URUS we are building the structure, tools, and skills to perform strong incident analysis so that we can identify systematic corrective actions to reduce risk for all our colleagues.  

Good Incident Analysis Parts
  • Incident investigation – Without fully understanding an incident, we won’t be successful in trying to prevent it from happening again. We are learning better techniques at investigating an incident from asking the right questions, to pictures, sketches, and collecting statements. This is our chance to play crime scene detective.
  • Incident Analysis – Here we ensure understanding of the incident, but more importantly we challenge ourselves in why something has occurred. Our new incident analysis form (linked below) has these two analysis tools built into it:

5 Why’s – This is a common and relatively simple way of trying to understand what has happened. By establishing a strong problem statement and asking “why” many times over, we can further understand contributing and causal factors. The purpose is to leverage a line of questioning to fully understand why something occurred.

  • Corrective Actions – The goal of our incident analysis is to find corrective actions that we can implement to prevent something from happening again. We often find immediate corrective actions such as communication opportunities, retraining opportunities, or quick fixes. We search for long term, systematic corrective actions that we can leverage to improve our business through risk reduction across URUS.
  • Reporting & CommunicationWhether communicating an incident or corrective actions, we will continue to improve how we communicate incidents up and down the organization to make all of us better. 
Good Incident Analysis Example 

Read the Safety Alert below that was the result of strong incident analysis at PEAK.  The site manager did an excellent job of investigating the incident and putting tools together for our small team to analyze the incident.  After a short discussion to ensure understanding, to challenge ourselves, and to identify contributing and causal factors we came away with some systematic corrective actions that will make our workplace safer.

  • We are building the incident reporting, incident analysis, and corrective actions processes within our Human Resources Information system (HRIS). This will streamline the overall process, standardize how we do things, and centralize it for improved tracking and sharing.
  • Once built, we will offer incident management and analysis training to our leaders to help communicate the new process and make roles and responsibilities clear.
  • We will share communication of incidents and corrective actions across URUS to be leveraged to take advantage of these learning opportunities.

Summary 

Learning from incidents that occur is a fundamental part of a great Safety program.  While we aim to be proactive in Safety to prevent incidents in the first place, safety incidents have a tremendous way of helping us learn where to focus.  Through good incident analysis many of our risk reduction opportunities will come to light, help us prioritize, and drive us toward the goal of sending everyone home the same way they came to us.
