Safety incidents present some of our best opportunities to improve. While we establish processes and systems to identify risk before an incident occurs, we would be remised if we didn’t do a great job of learning when an incident occurs. Throughout URUS we are building the structure, tools, and skills to perform strong incident analysis so that we can identify systematic corrective actions to reduce risk for all our colleagues.
5 Why’s – This is a common and relatively simple way of trying to understand what has happened. By establishing a strong problem statement and asking “why” many times over, we can further understand contributing and causal factors. The purpose is to leverage a line of questioning to fully understand why something occurred.
Read the Safety Alert below that was the result of strong incident analysis at PEAK. The site manager did an excellent job of investigating the incident and putting tools together for our small team to analyze the incident. After a short discussion to ensure understanding, to challenge ourselves, and to identify contributing and causal factors we came away with some systematic corrective actions that will make our workplace safer.

Learning from incidents that occur is a fundamental part of a great Safety program. While we aim to be proactive in Safety to prevent incidents in the first place, safety incidents have a tremendous way of helping us learn where to focus. Through good incident analysis many of our risk reduction opportunities will come to light, help us prioritize, and drive us toward the goal of sending everyone home the same way they came to us.
